Outbreak : plaque in Madagaskar!

From 23 August to 30 September 2017, a total of 73 suspected, probable, and confirmed cases of pneumonic plague, including 17 deaths have been reported in Madagascar. The diagnosis was confirmed by the Institut Pasteur de Madagascar by polymerase chain reaction test or using rapid diagnostic test.
The outbreak started following the death of a 31-year-old male from Ankazobe District in the Central Highlands (Hauts-Plateaux), a plague-endemic area. Since then, the Ministry of Public Health of Madagascar enhanced field investigations, contact tracing, surveillance, and monitoring all close contacts.

plaque is what you get on your teeth when you don't brush---I'm sure this is a big problem in Madagascar along with gum disease. Nonetheless, I think the word you want in the thread title is PLAGUE not PLAQUE.

Of these, 124 cases and 21 deaths (case fatality rate 16.9%) had the pneumonic form of the disease. Eight healthcare workers from one health facility in Tamatave have contracted pneumonic plague. Of 29 cases with specimens collected, 19 (66%) have been confirmed by either polymerase chain reaction (PCR) or RDT performed at the Institut Pasteur de Madagascar. On 3 October 2017, 37 new cases including 2 deaths have been reported....

So is this to be the usual ebola styled scare-mongering thread, replete with stupid graphics and constant updates putting the worst spin on things as possible, even as the crisis is clearly ebbing?
Given the track record of the perma-doomer blogs, I wouldn't bet a fast crash doomer's money on their predictions.

WHO provides 1.2 million antibiotics to fight plague in Madagascar

News release

6 OCTOBER 2017 | ANTANANARIVO, MADAGASCAR - WHO has delivered nearly 1.2 million doses of antibiotics and released US$1.5 million dollars in emergency funds to fight plague in Madagascar.

"Plague is curable if detected in time. Our teams are working to ensure that everyone at risk has access to protection and treatment. The faster we move, the more lives we save," said Dr. Charlotte Ndiaye, WHO Representative in Madagascar.

WHO has delivered 1 190 000 doses of antibiotics to the Ministry of Health and partners this week, and a further supply of 244 000 doses is expected in the days ahead.

The different types of drugs will be used for both curative and prophylactic care. They are enough to treat up to 5000 patients and protect up to 100 000 people who may be exposed to the disease.

The medicines are being distributed to health facilities and mobile health clinics across the country with the support of the Ministry of Health and partners.

WHO is also filling critical shortages in disinfection materials and personal protective equipment for health professionals and safe burials.

WHO and the Ministry of Health are training local health workers on how to identify and care for patients, and how to trace people who have had close contact with symptomatic patients so that they may be given protective treatment.

Most of the 231 infections and 33 deaths that the Ministry of Health has reported since August are associated with pneumonic plague – a more dangerous form of the disease that affects the lungs and is transmitted through coughing at close range.

Both bubonic and pneumonic plague can be cured using common antibiotics if delivered early. Antibiotics can also help prevent infection among people who have been exposed to plague.

WHO has rapidly released $1.5 Million from it emergency funds to allow for immediate support to the country until more substantial funds are received.

WHO is appealing for US$5.5 million to effectively respond to the outbreak and save lives.

More than a million doses of antibiotics have been delivered by the World Health Organization to fight an outbreak of plague in Madagascar which has killed at least 33 people.
The authorities have also banned prison visits in the two worst affected areas to prevent the spread of the disease.
The risk of contamination is high in overcrowded and unsanitary jails.
There has recently been criticism of the government's perceived slow reaction to the outbreak.

These were correlated against location information, with the result that the scientists were able to classify the strains into 18 geographically and genetically distinct subpopulations. These subpopulations have persisted for at least two decades.

Wagner and his team found that the geographically distinct plague groups were often carried by people from one area to another, but once transmitted seldom if ever established a new territory.

Thus, there is no single dominant strain of the bacteria on the island, which might make a useful target for any one-size-fits-all treatment. The team found that, instead, different geographic and genetic subtypes flared at different periods, causing outbreaks.......

The Minister of Health, Jean-Paul Adam, made the statement in the National Assembly on Wednesday when answering an urgent question from the Leader of the Opposition, Wavel Ramkalawan.

Adam was called to the National Assembly after the first case of pneumonic plague in Seychelles was confirmed on Tuesday. The patient is a man who returned to Mahe, the main island, last Friday, October 6 on board the last Air Seychelles flight from Madagascar.

The health minister also confirmed that 258 people are currently under observation and receiving treatment after the first case of pneumonic plague was detected.

“The 258 includes 41 passengers and seven crew from the flight, 12 close family members, and 18 staff and patients from the Anse Boileau health centre where the man went when he fell ill.”

The man disobeyed instructions given upon his arrival to remain at home for a period of time and attended a party that same evening. The minister explained that the result of the tests conducted on the infected man was sent to the Pasteur Institute in Paris, which confirmed the pneumonic plague.

Adam said that “all the 170 people who were at the party including the 13 staff working at the venue are also under passive surveillance.”

“We were already putting Seychellois and tourists under active observation at their homes or at hotels. But as from today, all those arriving from Madagascar will be put in isolation for six days,” the Minister said. ....

Madagascans battle to evade growing plague outbreak
October 11, 2017
The little footbridge near Justin Rakatoarivony's home is submerged in a murky green liquid the texture of sewage.

But he has no choice but to cross it every day on his way to work in Antananarivo, the capital of Madagascar.
The filthy conditions in his area, the southern Ampefiloha district, make him worry that he will be the next victim of the plague outbreak sweeping the country.
His fear is far from unfounded: the disease has already killed 25 people in Antananarivo since August, according to the health ministry.
"The plague is a disease that comes from the filth, because the filth attracts rats, and rats carry fleas which transmit the plague to humans," said Rakotoarivony, 45.
"I fear getting the plague here, but I don't have any choice, I have to cross this bridge every day, so I do it at speed."
Rakotoarivony is one of many on the Indian Ocean island nation who are increasingly fearful of the unusually virulent outbreak that has so far killed 54 people nationwide and infected 276 in the capital and its outskirts.
Everyone who crosses the bridge puts on a brave face despite the flow of brackish water and domestic rubbish beneath.
"The plague can kill in 12 hours," Edmond Rakotondrasoa, 46, a used-phone salesman, told AFP.
"But I'm not scared because I'm a believer and God wrote in the Bible, 'I will protect you from all epidemics'."
Scrap-metal merchants do business beside the accumulated detritus near the canal without a thought for the unhygienic surroundings.
Madagascar's outbreak includes bubonic plague, in which the germ Yersinia pestis is spread by infected rats via fleabites, and pneumonic plague, a particularly dangerous form which spreads from person to person via airborne droplets.
One of the traders there has already been diagnosed as a carrier of pneumonic plague.
The individual was hospitalised in a specialist clinic in Antananarivo—but discharged himself before completing his treatment.
'Spitting blood'
"Following our efforts to raise awareness about the plague, a group brought in a 24-year-old man who was spitting blood," said Rabenjaminahobianintra Harimanana, head doctor at the Isotry Central health clinic.
"Tests (for plague) came back positive, so after being given initial treatment he was sent to the Ambohimiandra Anti-plague Clinic.


"But he escaped from the centre and was seen once again trading in the market."
Authorities sought to reason with him, but he fled again.
He was declared a wanted man but is still at large.
"He was scared because he was a repeat offender who had been released from prison," said Harimanana.
"We alerted the police so they can return him to hospital if they find him."

Hanitra Randrianarison, a senior medical official in Antananarivo, said "this fugitive patient risks spreading the disease very quickly to anyone who gets within two metres (six foot) of him."
President Hery Rajaonarimampianina on Tuesday described tackling the plague in military terms.
"We are at war... and today I think we have the weapons and munitions to fight this epidemic," he said in his first public comments on the crisis as he visited affected areas.
The current crisis was sparked on August 28 after a single fatality in the central town of Ankazobe. The victim, a passenger in a public taxi, passed away as the vehicle travelled across the country.
They infected two other passengers who died at the beginning of September in Tamatave, a town on the island's east coast that had gone for 100 years without a single recorded instance of the plague.
Now some 500 cases of plague have been identified nationwide, prompting authorities to take drastic measures.
'A baseless rumour'
Passengers at Antananarivo's transport hubs are subject to medical inspections, infected areas have been fumigated to kill fleas, public gatherings are banned, and schools and universities have been shut.
"Local health inspectors are working to identify people infected with the plague and to inform people about the importance of hygiene and cleanliness," said Randrianarison.
"All of the local communities in Antananarivo have been taking part in a city-wide clean-up drive."
This week, the health ministry trained 384 local health inspectors to deal with plague in Antananarivo.
Among them was Norosoa Raharimalala, who works in Ivandry district in the capital's north.
Unlike some of those who live and walk alongside the filthy Andriantany canal, the people of Ivandry have been panicked by the plague outbreak, according to Norosoa.
They no longer shake hands to greet one another, and avoid standing close to each other.
"It takes a plague for people in our area to appreciate cleanliness and hygiene," said Raharimalala.....

A South African basketball player who had contracted plague in Madagascar has been successfully treated.

The player has returned to South Africa and the National Institute for Communicable Diseases (NICD) on Thursday issued a statement saying the player does not pose any health risk.

Details about the player and the team have not been established yet by TimesLIVE but the NICD is in contact with the group and has mentioned that it will follow up on them.

The NICD warned South Africans travelling to Madagascar to be highly cautious of their surroundings as the country is experiencing an outbreak........

CAPE TOWN - The National Institute for Communicable Diseases says there are plans in place should the pneumonic plague reach South Africa.

More than 340 people have been infected and 42 have died following an outbreak of the disease in Madagascar.

The institute's John Frean explains: “We’re prepared for the eventuality. Preparations are being mad to cope with that possibility.”

The pneumonic plague is described as a severe lung infection.

The Centre for Disease Control and Prevention says that people should look out for symptoms like fever, headache, weakness, a rapidly developing pneumonia with shortness of breath, chest pain, cough and sometimes bloody or watery mucous.

In a follow-up on the Madagascar plague outbreak, the Ministere de la Publique reported an additional 51 news plague cases today, bringing the total cases since August to 500. On Oct. 11, 29 new pneumonic plague cases were reported, while and additional dozen bubonic plague cases were reported. 10 suspect cases were also recorded.


In addition, six additional deaths have been reported, bringing the total to 54. The capital city of Antananarivo saw an additional 37 cases, mostly pneumonic, and four deaths.

“Pneumonic plague is a form of plague that is transmissible from person to person, with a potential to trigger severe epidemics if inadequately controlled,” the organization said in a statement.

It said there was also a moderate risk the disease could spread to other countries in Africa, because there are frequent flights to nearby countries and Indian Ocean islands.

But according to the WHO, the danger of the plague spreading around the globe is low.

With dozens dead from a plague outbreak in Madagascar, the International Federation of Red Cross and Red Crescent Societies said Friday it is deploying its first-ever plague treatment center to the island nation.
The World Health Organization said 561 cases have been reported so far, with another in the Seychelles.[/color]

Red Cross officials have said the situation is particularly worrying because pneumonic plague, which is spread from person to person, has occurred for the first time in non-endemic areas and crowded cities. About 70 percent of the cases are the pneumonic form.

The plague outbreak in Madagascar continues to grow as the countries Health Ministry reported Thursday that 35 Districts were now affected by the plague epidemic.

As of Oct 12, 561 reported cases of plague were recorded, 415 of which were pulmonary, 145 of bubonic forms and another case that is unspecified, according to a Flash Bulletin published yesterday.

Of this total, 10.2 percent, or 57 fatalities have been reported.

A meeting was held yesterday at the Prime Minister’s Office in the presence of the partners from humanitarian organizations (WHO, USAID, UNICEF, etc.). It was pointed out that 9.9 million Euros is needed to support the fight against spread of plague at the country level.

On October 11, 2017, the USA, through USAID, donated to the State of Malagasy a number of items to help in the fight against plague to include 8 mobile clinic vehicles (6 for Antananarivo and 2 for Toamasina), gloves, masks and water.

Seychelles schools closed over plague fears

The Seychelles government ordered schools to close Friday, after the discovery of two suspected cases of plague thought to have been brought from Madagascar where the disease has killed scores.

The health ministry has also put under surveillance 320 people who have come into contact with the two patients.


A total of 12 people showing plague-like symptoms have been admitted to hospital and given antibiotics.

Panic gripped parents on the Indian Ocean archipelago after some students developed fevers in recent days, leading to the school closures.

"We made this decision as a precautionary measure to reassure parents," said Merida Delcy, an adviser to the education ministry, noting that they would not reopen until Wednesday at the earliest.
